Communities are shaped by far more than individual choices. The conditions where people are born, live, learn, work, and age profoundly influence community health outcomes. This session explores the Social Determinants of Health (SDOH) through an asset-based lens, equipping professionals with the knowledge and tools to assess community health needs, leverage existing resources, and strengthen community health impact. Using interactive mapping tools and the Social Ecological Model as a framework, participants will learn to identify not just gaps, but community strengths.
Learning Objectives:
- Identify the five domains of Social Determinants of Health (SDOH) and explain how environmental and structural conditions influence health outcomes across the lifespan.
- Apply the Social Ecological Model (SEM) to analyze community health challenges and recognize how factors beyond individual control shape health behaviors and outcomes.
- Utilize SDOH data tools to assess community health needs and identify existing assets, resources, and gaps within a defined geographic area.
